Forms that need to be completed and handed in to Cindy Koldewey:
1. USA Hockey Registration - Online at www.usahockey.com - most of you have already done this and sent them to me. If you completed your registration online, you already checked the box for the USA Waiver of Liability and USA Assumption of Risks forms and won't need to fill out these forms.
2. Player and Parent Information Sheet - can download from this page
3. USA Hockey Participant Code of Conduct - can download from this page
4. USA Hockey Consent to Treat - can be downloaded from this page
5. Canlan Player Liability Form - can be downloaded from this page
6. Academic Eligibility Verification Form - has to be taken to school and signed. If your school refuses to sign these, you can use your final report card from last year. Must be signed by the parent. These need to be returned to me by September 20th. Freshmen do not need these forms until after their first grading period is completed.
